I have been at work since Sunday night. We have had a snow storm so I have been here, unable to go home. I slept some yesterday and today. I actually wrote a big long post about this this morning but it seems to be gone now. I just wanted to talk with you about it because I think you are the only people who will understand.
I had a patient come in last night to the other hospital. They are not supposed to do that, but it happens. So the nurse called to tell me what was going on. She was 21-6/7 weeks with vaginal bleeding and contractions. I tried to get over to the other hospital but my car doors were frozen shut. I went back inside and asked if they had anyone in house that could do an ultrasound for me while I tried to figure out how to get there. So while they were doing the ultrasound, they called back and said that they could not even measure her cervix because the membranes were all way through the external os.
So, the other doctor on with me and I went back outside with spatulas from the kitchen and a pitcher of warm water and thawed mt doors enough to get in and drive over there. Sure enough when I checked her with the speculum she was completely dilated with her membranes bulging through. I explained to her that there really is nothing I can do. She was contracting every 3 minutes and completely dilated, and at 21 weeks this little one would not be able to survive. She knew all too well what I was saying since she had just delivered a little boy at 20 weeks last year.
So we got her tucked in and I had to go back to the other hospital again to take care of those patients. I had not even back an hour when they called to tell me she felt like pushing and that they coud see membranes bulging at the introitus.
I got back to her as soon as I could. When I arrived she was pushing and this bag of water was bulging quite a bit, so I decided to go ahead and rupture her so that we could get her out of pain and delivered. When I did, that little baby was kicking and playing with my hand. She was breech.
The baby was finally delivered. The parents did not want to see or hold her or anything, so we took her to a separate room to hospice her. I had to stay until the baby died since I was the one who had to pronounce her when she died. That little baby held on for 55 minutes.
I don't think if I do this 100 years I will ever get to the point that this does not bother me. It just makes my chest hurt. The nurse and I took turns holding her while she tried to take a few breaths. I did my exam on her...she was perfect.
Just broke my heart....
So, Im back on the board in couple of hours. I have been in this hospital for 48 hours. I am going to try to drive the roads and go see my family for dinner before I have to be back.
